Royal Malaysian Navy Launches Investigation into Tragic Helicopter Collision
Ten individuals tragically lost their lives Tuesday in a mid-air collision between two Royal Malaysian Navy (RMN) helicopters during a rehearsal for the navy’s 90th-anniversary celebration.
Preliminary Findings
According to the RMN, an AW139 maritime operation helicopter carrying seven crew members and a Fennec lightweight helicopter with three crew members were involved in the incident. All ten personnel aboard perished. The wreckage of the AW139 was discovered on a sports complex within the naval base, while the Fennec crashed near a swimming pool.
Investigation Process
The RMN has established an “Investigation Board” to determine the cause of the collision. The board will meticulously examine all available evidence, including eyewitness accounts, radar data, and physical debris.
The probe will delve into several key areas:
- Mechanical malfunctions: Inspecting the helicopters for any prior defects or maintenance issues that may have contributed to the incident.
- Human factors: Examining the actions and decisions of the pilots and crew to identify potential errors or misjudgments.

“We understand the gravity of this tragedy and are determined to uncover the truth,” said Admiral Mohd Reza Mohd Sany, Chief of Naval Staff. “The families of the victims and the Malaysian public deserve nothing less.”

The investigation will also seek to extract valuable lessons to prevent similar incidents in the future. The RMN intends to implement safety enhancements based on the board’s recommendations.
“This tragedy serves as a harsh reminder of the inherent risks associated with military aviation,” said Defense Minister Mohamed Khaled Nordin. “We will use this opportunity to reassess our procedures and ensure that our personnel operate in the safest possible environment.”

The RMN is considering seeking assistance from international experts and organizations with experience in marine helicopter operations and accident investigations. This collaboration will provide valuable insights and expertise.

The mid-air collision has sent shockwaves throughout Malaysia and the international community. Prime Minister Anwar Ibrahim expressed his deepest condolences to the families of the victims and ordered a full investigation.
